Feedback and Suggestions Form

Your comments and suggestions are important to us - please feel free to complete one of the feedback and suggestions forms that are always available in the downstairs waiting room at the surgery.  

Please note : only use this form for comments about the practice and suggestions as to how we can improve our service to you.

Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

Complaints

We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can sometimes go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is the case, we would wish for the matter to be settled as quickly and as amicably as possible.

To pursue a complaint please contact the Practice Management Team who will deal with your concerns appropriately. Further written information is available regarding the complaints procedure from reception.

Most problems can be sorted out quickly and easily, often face to face at the time they arise with the person concerned,  and this may be the approach you try first. We aim to settle complaints as soon as possible.

PALS (Patient Advice and Liaison Service)

The Patient Advice and Liaison Service (PALS) offers confidential advice, support and information on health-related matters. They provide a point of contact for patients, their families and their carers.

How can PALS help?

PALS provides help in many ways. For example, it can:

  • help you with health-related questions
  • help resolve concerns or problems when you're using the NHS
  • tell you how to get more involved in your own healthcare

PALS can give you information about:

  • the NHS
  • the NHS complaints procedure, including how to get independent help if you want to make a complaint
  • support groups outside the NHS

PALS also helps to improve the NHS by listening to your concerns and suggestions.

How do I contact my nearest PALS?

You can find your nearest PALS office online here or ask the surgery, local hospital or phone NHS 111. 

Making a complaint

If you're not happy with an NHS service, you should complain to the person or organisation providing the service in the first instance, such as the GP, dentist, hospital or pharmacist. Alternatively, you can complain to the commissioner of that service – either NHS England or the area clinical commissioning group (CCG).


In general, NHS England commissions most primary care services, such as GP and dental services. CCGs oversee the commissioning of secondary care, such as hospital care and some community services.
