Sickness certification

The Government’s rules around issuing a fit note (sometimes called a 'sick note' or a MED3 form) are that a medical professional can only issue a MED3 when an individual has had more than seven continuous calendar days off sick due to an illness (including weekends). For any period of illness less than seven days, you can complete a self-certification certificate.

For information about sick leave and self-certification, see 'Taking sick leave' on Gov.uk.

If your employer requires you to complete an SC2 form for Statutory Sick Pay you can find this here.
